    Update on the ICUD-SIU consultation on multi-parametric magnetic resonance imaging in localised prostate cancer

    Introduction: Prostate cancer (PCa) imaging is a rapidly evolving field. Dramatic improvements in prostate MRI during the last decade will probably change the accuracy of diagnosis. This chapter reviews recent current evidence about MRI diagnostic performance and impact on PCa management. Materials and methods: The International Consultation on Urological Diseases nominated a committee to review the literature on prostate MRI. A search of the PubMed database was conducted to identify articles focussed on MP-MRI detection and staging protocols, reporting and scoring systems, the role of MP-MRI in diagnosing PCa prior to biopsy, in active surveillance, in focal therapy and in detecting local recurrence after treatment. Results: Differences in opinion were reported in the use of the strength of magnets [1.5 Tesla (T) vs. 3T] and coils. More agreement was found regarding the choice of pulse sequences; diffusion-weighted MRI (DW-MRI), dynamic contrast-enhanced MRI (DCE MRI), and/or MR spectroscopy imaging (MRSI) are recommended in addition to conventional T2-weighted anatomical sequences. In 2015, the Prostate Imaging Reporting and Data System (PI-RADS version 2) was described to standardize image acquisition and interpretation. MP-MRI improves detection of clinically significant PCa (csPCa) in the repeat biopsy setting or before the confirmatory biopsy in patients considering active surveillance. It is useful to guide focal treatment and to detect local recurrences after treatment. Its role in biopsy-naive patients or during the course of active surveillance remains debated. Conclusion: MP-MRI is increasingly used to improve detection of csPCa and for the selection of a suitable therapeutic approach

    The role of magnetic resonance imaging (MRI) prior to biopsy in the diagnosis of prostate cancer in biopsy-naĂŻve patients has been strengthened by recent developments such as the PIRADS V2 criteria, which cover acquisition, interpretation, and reporting for clinical practice and data collection for research. Important questions on the role of prostate MRI remain: can MRI be used as a triage test before first biopsy series? Can it be used to avoid the use of systematic biopsies (SB) and instead use only targeted biopsies (TB) to MRI-suspicious lesions? Studies to evaluate image guided TB compared to SB have started to accumulate. Objectives of these studies should be to reduce the detection of clinically insignificant disease, to maximize the detection of clinically significant cancer (CSC), to better assess disease size, grade and location. Accurate diagnosis will allow the choice of the most appropriate treatments options, minimising side effects and reducing overtreatment. Study results on MRI-TB detection rates are promising however some limitations should be considered. The majority of published and ongoing studies have been performed at expert centres, in order to demonstrate the optimal performance of MRI. Then, the validation of this strategy in less specialised institutions will be necessary before incorporating recommendations in international guidelines. It necessitates training for radiologists and urologists to perform and read MRI and MRI-targeted biopsy through education programs and standardization tools. All these advances will be consolidated with expected genetic screening tools to improve the detection of aggressive cancer

    Influence of secondary emission yield on the saturation properties of multipactor discharges between two parallel metal plates

    A detailed numerical simulation analysis is made of the saturation stage of multipactor discharges between two infinite parallel metal plates in vacuum exposed to a rf voltage. The main physical effect causing saturation of the multipactor discharge is increased space charge, as the electron density becomes large. It is found that the properties of the saturation stage depend crucially on the value of the secondary emission yield of the metal surfaces. Below a certain threshold value, the discharge has a two-sided character, but at this threshold the discharge makes an abrupt transition into two decoupled single-sided multipactor discharges containing significantly increased electron densities. The result of the numerical simulations gives a good picture of the saturation properties of the multipactor discharge and is also supplemented by an approximate analytical investigation that highlights and explains the characteristic properties of the observed saturation behavior

    Assessing optical earth observation systems for mapping and monitoring temporary ponds in arid areas

    Remote sensing methods for locating and monitoring temporary ponds over large areas in arid lands were tested on a study site in Northern Senegal. Three main results are presented, validated with field data and intended to highlight different spectral, spatial and temporal characteristics of the methods: (1) Among several water indices tested, two Middle Infrared-based indices (MNDWIModified Normalized Difference Water Index and NDWI1Normalized Difference Water Index) are found to be most efficient; (2) an objective method is given prescribing the necessary sensor spatial resolution in terms of minimal detected pond area; and (3) the potential of multi-temporal MODIS imagery for tracking the filling phases of small ponds is illustrated. These results should assist in epidemiological studies of vector-borne diseases that develop around these ponds, but also more generally for land and water management and preservation of threatened ecosystems in arid areas

    Simple model of the rf noise generated by multipacting electrons

    A simplified analytical model is developed to predict the spectrum of electric current induced by the multipacting electrons between two parallel electrodes exposed to an rf voltage of fixed amplitude. The model is based on the resonant multipactor theory and makes it possible to calculate the relative spectral amplitudes of electric current at different harmonics and sub-harmonics of the applied rf frequency. The theoretical predictions are confirmed by numerical simulations of multipactor inside a rectangular waveguide. Specifically it is seen that the relative height of the spectral peaks decreases with increasing gap height

    Low-risk prostate cancer selected for active surveillance with negative MRI at entry: can repeat biopsies at 1 year be avoided? A pilot study

    Purpose: In patients considered for active surveillance (AS), the use of MRI and targeted biopsies (TB) at entry challenges the approach of routine “per protocol” repeat systematic biopsies (SB) at 1 year. This pilot study aimed to assess whether an approach of performing repeat biopsies only if PSA kinetics are abnormal would be safe and sufficient to detect progression. / Methods: Prospective single-centre study of 149 patients on AS with low-risk PCa, a negative MRI at entry, followed for a minimum of 12 months between 01/2007 and 12/2015. Group 1 (n = 78) patients had per-protocol 12-month repeat SB; group 2 (n = 71) patients did not. Surveillance tests for tumour progression were for both groups: for cause SB and MRI-TB biopsies if PSA velocity (PSA-V) > 0.75 ng/ml/year, or PSA doubling time (PSADT) < 3 years. The main objectives are to compare the 2-year rates of tumour progression and AS discontinuation between groups. The secondary objectives are to estimate the diagnostic power of PSA-V and PSA-DT, to predict the risk of tumour progression. / Results: Overall, 21 out of 149 patients (14.1%) showed tumour progression, 17.1% for group 1 and 12.3% for group 2, and 31 (21.2%) discontinued AS at 2 years. There was no difference between the 2 groups (p = 0.56). The area under the PSA-V and PSADT curves to predict tumour progression was 0.92 and 0.83, respectively. / Conclusions: We did not find any significant difference for progression and AS discontinuation rate between the 2 groups. The PSA kinetic seems accurate as a marker of tumour progression. These results support the conduct of a multi-centre prospective trial to confirm these findings

    The potential for remote sensing and hydrologic modelling to assess the spatio-temporal dynamics of ponds in the Ferlo Region (Senegal)

    In the Ferlo Region in Senegal, livestock depend on temporary ponds for water but are exposed to the Rift Valley Fever (RVF), a disease transmitted to herds by mosquitoes which develop in these ponds. Mosquito abundance is related to the emptying and filling phases of the ponds, and in order to study the epidemiology of RVF, pond modelling is required. In the context of a data scarce region, a simple hydrologic model which makes use of remote sensing data was developed to simulate pond water dynamics from daily rainfall. Two sets of ponds were considered: those located in the main stream of the Ferlo Valley whose hydrological dynamics are essentially due to runoff, and the ponds located outside, which are smaller and whose filling mechanisms are mainly due to direct rainfall. Separate calibrations and validations were made for each set of ponds. Calibration was performed from daily field data (rainfall, water level) collected during the 2001 and 2002 rainy seasons and from three different sources of remote sensing data: 1) very high spatial resolution optical satellite images to access pond location and surface area at given dates, 2) Advanced Spaceborne Thermal Emission and Reflection Radiometer (ASTER) Digital Elevation Model (DEM) data to estimate pond catchment area and 3) Tropical Rainfall Measuring Mission (TRMM) data for rainfall estimates. The model was applied to all ponds of the study area, the results were validated and a sensitivity analysis was performed. Water height simulations using gauge rainfall as input were compared to water level measurements from four ponds and Nash coefficients &gt;0.7 were obtained. Comparison with simulations using TRMM rainfall data gave mixed results, with poor water height simulations for the year 2001 and good estimations for the year 2002. A pond map derived from a Quickbird satellite image was used to assess model accuracy for simulating pond water areas for all the ponds of the study area. The validation showed that modelled water areas were mostly underestimated but significantly correlated, particularly for the larger ponds. The results of the sensitivity analysis showed that parameters relative to pond shape and catchment area estimation have less effects on model simulation than parameters relative to soil properties (rainfall threshold causing runoff in dry soils and the coefficient expressing soil moisture decrease with time) or the water loss coefficient. Overall, our results demonstrate the possibility of using a simple hydrologic model with remote sensing data to track pond water heights and water areas in a homogeneous arid area

    Structure and rheological properties of model microemulsion networks filled with nanoparticles

    Model microemulsion networks of oil droplets stabilized by non ionic surfactant and telechelic polymer C18-PEO(10k)-C18 have been studied for two droplet-to-polymer size ratios. The rheological properties of the networks have been measured as a function of network connectivity and can be described in terms of simple percolation laws. The network structure has been characterised by Small Angle Neutron Scattering. A Reverse Monte Carlo approach is used to demonstrate the interplay of attraction and repulsion induced by the copolymer. These model networks are then used as matrix for the incorporation of silica nanoparticles (R=10nm), individual dispersion being checked by scattering. A strong impact on the rheological properties is found for silica volume fractions up to 9%
